Q&A
Do you need three copies of the certification that has to be notarized and conflict of interest form along with the RFP?
Yes, three notarized copies of the Certification and signed Conflict of Interest forms are required with the RFP submission.
Also, it is requested that a digital pdf copy be included along with the mailed copies, and I wanted to make sure that is intended to be emailed to you and not a thumb drive?
A digital PDF copy is also required and should be provided on a thumb drive included with the three mailed copies (not by email).
Can the limits of professional liability insurance be decreased or removed as a requirement of the proposal?
The update to our MSA, based on the advice of legal counsel, may be discussed at the same Board meeting under the same agenda item as the
contract award. At this time, this is only a suggestion and has not yet been confirmed. Also, Section 7.3 of the MSA states that the Board, or its designee, has the sole discretion to increase or decrease insurance coverage requirements in connection with any Task Orders, with the understanding that the Inspector may adjust the amount charged accordingly.

Is Construction Management and Construction Contract Administration included in the scope of this RFP?
The scope is limited to observing and documenting construction activities to verify that they are performed in accordance with the approved
engineering plans and specifications, monitoring the project schedule, and reporting any deviations from the plans or specifications to the engineers and to us.
As posted on CIVCAST, ‘three notarized copies of the Certification and signed Conflict of Interest forms are required with the RFP submission.’ The assumption is this one set of each document per hardcopy. Correct?
